The following two (2) forms are provided and may be printed, filled-in and returned to City Hall.
The first form is used to address a concern, complaint, or comment. Please fill-in all requested information and return it to City Hall by: mail, fax or drop-off.

The second form is used if you wish to receive a "follow-up" or reply to your concern, complaint, or comment. Please note that due to privacy issues we may not be able to furnish you with complete details of our actions but we can inform you that your issue was addressed. Additionally, please be aware that all requests are subject to the Freedom of Information Act and may need to be provided, if requested.
